Virtual Doctors Recomendation

This is a standalone virtual doctors recommendation, this does not include any service related to anything but the recommendation. If you go this route you will need to fill out everything else yourself and submit it to OMMA. The doctor will call the phone number that you list and will issue a recommendation based on their opinion. Once you purchase you will receive a link to a short form you will need to fill out for the doctor, make sure you select doctors recommendation under the service tab and it will guide you through. You will need a valid email address and a picture of your drivers license.
